Whether you are a returning family or new to our school, our team will be available to assist you through the enrollment process and ensure everything is completed smoothly.

  1. Proof of Residency in the District of Columbia
  2. Parent ID (Driver’s License)
  3. Birth Certificate
  4. Immunization Records
  5. DC Universal Health Certificate
  6. DC Oral Health Assessment Form
  7. Medication and Medical Procedure Treatment Plan /Asthma Action Plan (If Applicable)
  8. Custody Documents (If Applicable)
  9. IEP/504 Plan (If Applicable)
