Dear Parent/Guardian:
Harmony DC has partnered with the Prevention of Blindness Society of Metropolitan Washington to provide vision screenings for your child.
Your child’s vision will be screened on Thursday, April 2nd and Friday, April 3rd.
There is no cost to you for this program. If your child wears prescription glasses, please have them bring them to school on the day of the screening.
The POB vision screening program is responsible for scheduling and professionally administering the tests and referring children who are identified with a vision problem.
If your child fails the vision screening, you will receive another form from the school asking for information, and they will be included in a follow-up vision clinic that we will host at your child’s school, where they will see a doctor and, if needed, be prescribed glasses.
The program is conducted under the guidelines established by POB’s Medical Advisory Board and local health officials.
